The Guardian's technology section is looking to create an interactive map of UK technology start-ups in an attempt to attract VC spend and help it create news stories.

If you work for one, or know one, the Guardian would like to take up 2 minutes of your time and ask you to complete a form.

You can enter your information if your company (a) is up to five years old (b) employs fewer than 100 people (c) has a turnover/revenue/sales less than £10m.
